The SNP is a program that provides meals to older adults across the County of Santa Clara. Meals are delivered through two main platforms: congregate meals (in community) and home-delivered meals. The typical SNP customer is an older adult, mostly people 75 and over (~53%), living alone (~30%), living below the federal poverty level (~50%), and part of a minority population as defined by the Older Americans Act of 1966 (~70%). These clients often face language barriers, face transportation difficulties, are impacted by other nutritional deficiencies, and difficulty accessing community resources that keep them healthy.
SNP is a program that provides nutrition services but is also a conduit to vital programs such as legal services, Health Insurance Counseling Assistance Program, peer advocate, health services such as dental and medical, and social activities. Clients would access information through direct contact with the sites and the website. However, on the website platform the information gets updated once a year and doesn’t provide information on transportation, reservation systems, and dates/times of services. By developing the Senior Nutrition Program Food Map Web Application, increase access to vital services for older adults in the community that include food access resources. People would, ideally, be able to enter their home address or desired target zip code and get a list of nearby locations, bus routes, driving/walking directions, days of service, hours of operations and other attributes. This web application is to be used by the entire community including older adult, adult child, grandchild, caregiver, provider, and community stakeholder by accessing personal computer or the mobile device.
Functional features of the application include the following:
- Application is responsive and can be accessed via mobile and desktop devices.
- Allows users to search locations where they can go to get SNP services.
- Users are able to get turn by turn directions, information on hours of operations and other various attributes.
- SNP content owners are able to access an administrative portal to update data.
Technical features of the application focused on cloud computing building the application the Azure platform using:
- Cosmos DB
- Azure Search
- Azure Map
Currently, the team including developers and stakeholders are working on refining the user interface and perform internal testing to ensure an optimal user experience and meet project goals. The scrum agile methodology is used for the development of this application and the anticipated implementation date for this application is to go-live is before March 2020.
